As a teacher, you are usually the person responsible for detecting early symptoms of ADHD and other behavioral problems and are considered a valuable source of information about how a child is doing during treatment.

I know It’s tough enough handling a bunch of different personality children for seven hours a day and trying to make sure they are learning according to state and federal guidelines. But, when you add one or two kids to that same class with behavioral problems such as ADHD, a teacher’s workload can more than double. It takes a special person to dedicate their life to improving others and society as a whole. Thank you!

Likewise, I’m sure it’s really hard to keep up with little changes in each of your students. Trying to give input to parents and doctors every month or so can probably get very hectic due to your overwhelming workload. So, I imagine when you finally have time to sit down and really assess each student; it’s already half way through the year.

Most teacher assessment forms require too much input and the questions you’re asked to answer are too vague. The goal in providing my “short form” is to allow you a more focused input into your student’s treatment and progress. You’ll find the questions do not require you to gauge or assign a number to behaviors and therefore should be easier to use.

Please feel free to explain any response as needed and offer advice. Teachers are a treasure trove of knowledge. I know how valuable each and every one was to my education and success as a doctor. “Teachers change the future one mind at a time.”

Now before we continue to the assessment page…
I’d like to offer an easier more efficient way for you to provide that invaluable input about how your student and our patient is doing with his or her treatment. Feel free to ask me any question or give me feedback on-line at drfrank@drhuggiebear.com . Simply put “teacher question (feedback) from (your name)” in the subject line. Please mention your student by “girl or boy” only and no other identifying attribute. Submit your question or input and let the child’s parents know you have sent me a message. Then they can let us know which e-mail is about their child!
Now on to the short form…
