Drug Update   Using St. John’s wort for Depression

The herb St. John’s wort (Hypericum perforatum L.) has been used as a medication for over 2000 years. Recent studies have shown it to be useful in the short-term treatment of mild depression. Doctors consider short term to be less than 6 months. St. John’s wort does not mix well with some medications and should not be taken with prescription antidepressants or ephedra (MaHuang). In combination, these drugs may cause irritability, agitation, headaches, nausea, photosensitivity, and worsening of depression.

Parents and teachers are the best source of information about a child’s behavior. Even older children often cannot describe their own behavior. In order for your doctor to make an accurate diagnosis and then gauge how well treatment is going, you should provide input from both of these sources.

Tick Bites and Fevers       Frank Barnhill M.D.

As warm weather approaches, we all have to be concerned with tick bites and the diseases they can carry. Tick bite illnesses can occur almost anytime during the year, but are most often seen in spring and summer. Common names for these afflictions include Rocky Mountain Spotted Fever, Lyme Disease, Ehrlichiosis (er-lick-ee-o-sis), Tularemia (too-la-ree-me-a), and Babesiosis (bab-e-see-o-sis).

A germ carried by deer, dog or other area specific ticks causes each of these diseases. For example, Lyme Disease is more common in the deer tick of northeastern and midwestern United States, but can also be found in all other states as well as Asia, Australia, and Europe. In contrast, Babesiosis tends to be most commonly found in deer ticks in Connecticut, Massachusetts, and New York.

Most of us are exposed by walking through grassy areas with low shrubs, as ticks wait on the outer branches or leaves for a warm bodied person or animal to brush up against them. Even if they don’t land on skin, they will cling to clothing and crawl for hours to find a place to attach.

Despite the fact that each disease is a separate illness detectable by blood testing, they all share common symptoms. Most of us who’ve had one of the diseases felt like we had an early case of the “flu”. So, it’s common for those affected to “ache all over”, feel tired, have a fever, chills, cough, or sore throat, notice a headache, have an upset stomach or break out in a rash. It’s important to understand that tick bite rashes occur in less than thirty percent of all those bitten. So, don’t rely on the appearance of a rash to decide whether to go see a doctor or not. A good rule in any sickness is “If flu symptoms hang around for more than four or five days, then it’s usually not the flu”.

The rash of Lyme Disease occurs with the bite and is called erythema migrans. It starts as a red spot and may enlarge to form a “bull’s-eye”, or spread all over the body. Those afflicted with Lyme Disease may be a little sicker early on. Since the disease can affect the nervous system, you can have a headache with a stiff neck, a slow or irregular heartbeat, and more prominent joint and muscle pains.

How do you know if you have one of these illnesses? Your Family Doctor will do an examination and draw blood for testing for each disease. Sometimes, you can have the disease, but blood tests fail to show it present. In such cases, if your doctor suspects the disease anyway, it will be necessary to take one or two antibiotics and repeat your blood tests in about sixty days. Unfortunately, if you are treated at an early stage in your illness, blood work may never show that you had the disease. A word of caution: if you or your doctor are suspicious the disease is present, then you should receive treatment even with negative blood tests. If untreated, these tick borne diseases can cause serious damage to different parts of the body, as in Lyme Disease nerve damage, as well as death.

What can you do to avoid the disease and keep your children safe?
  1. Avoid areas where ticks nest between May and October
  2. When outdoors in grassy or wooded areas, wear long sleeves, long pants and tuck pant legs into boots or socks to prevent ticks from getting to bare skin
  3. Always check yours and your children’s clothing for “roaming ticks” when coming in from play or outside activities (When I work in the yard, I brush off and shake all of my clothing before going into the house, since ticks may “roam” for hours before attaching) Wearing light colored clothing helps you spot the little critters easier!
  4. Have your partner check you and you check your kids from head to toe once a day for the presence of attached or roaming ticks.
  5. Apply DEET containing tick repellants to skin or clothing before going into wooded or grassy areas. Do not use permethrin repellants directly on skin and always avoid applying anything to areas around the eyes and on the hands of small children
  6. Remember that ticks usually do not transmit any disease until they have been attached for several hours. Do not try to burn, suffocate or irritate an embedded tick. If you do so, you stand a greater chance the tick will regurgitate and transmit the disease directly into the bite wound
  7. Remove embedded ticks as soon as possible by using a pair of tweezers to firmly grasp the head and pull all body parts out. (See article) Clean the bite area with alcohol and apply Neosporin.
  8. Mark your calendar with the date you discover an embedded tick, as most symptoms will show up within three weeks.

  “Can you hear me now?”

“Team Meetings Are A Waste Of Time”    by Frank N. Jones

So, your boss sent a memo to everyone in your department to let you all know the next team meeting would be a week from Tuesday at 10:00 AM. And just to make matters worse, there was a scribbled note at the bottom "suggesting everyone attend." Honestly, the first reaction you probably had was just plain dread and despair. "Another two hours wasted again, and nobody learns anything new." You probably bounced it at a fellow worker; "Doesn't he know we're all too busy for his so called team meetings?"

The conversation probably continued with you explaining the boss always comes up with ideas that we've heard over and over, or stuff that just won't work. He'll "preach" for a couple of hours, and even if you don't nod off, he won't really say anything of any worth. You added everyone had agreed many times in the past that these "team meetings" did more harm than good. They just seemed to kill morale and be a total waste of time.

During lunch with a competing group of folks in the film business, a junior sales person related his frustrations about “worthless meetings” and how he inadvertently directed his feelings toward his secretary. As a new employee of his company, he felt frustrated and a little angry that his boss wanted more productivity, but wasted time he felt could be used more successfully.

This wasn’t the first time I had seen or heard such dread, gloom and doom at this level. It’s a common feeling in mid-range to large corporations. He was obviously feeling guilty and was looking for a sympathetic ear. I explained he wasn't in this boat alone, and that what he said seemed to highlight most of today’s “time stressed” employees' feelings about meetings.

Trying to offer advice without seeming out of place, I told him how our company vice president handled team meetings and the system we had in place to help assure meetings were indeed productive. Since my new young friend seemed talented, I explained how he could help change this “waste of time" into a highly productive event by doing a little homework before each meeting.

I advised he stop and ask, "Can I really make a difference?" The answer is almost always, "Yes!" When you learn of the team meeting, spend time coming up with just one real good suggestion for making your work place better. Don't try to spring it on your boss during the meeting, instead make notes and think out your suggestion beforehand. When you've got it all together, ask to meet with your boss a day or two before. Explain the company benefits if your suggestion were discussed and placed in use. Don't try to impress him or her with a long list of suggestions. Just go with the one that you think is the best. Simply ask if he had ever looked at this opportunity from ''this'' point of view, carefully laying out how it's handled now, and the benefits you feel he and the company would achieve if changed. Remember, you have to get him involved if you expect change. And the secret to getting his endorsement is to present the idea where he can feel comfortable with your recommendations. That way if he doesn’t buy into your ideas, neither of you will be embarrassed. If he agrees in the value of your suggestion, he will ask you to present your thoughts during the meeting.

As I explained to the "new guy," we were in bankruptcy and really using every creative tool to save our company and of course our jobs. I had used this very formula in an important team budget meeting. About a week beforehand, I privately approached my new boss, asking him if he had time to sit down for a few minutes. We all knew we had to watch every penny in order to keep the company going and our jobs secure. I suggested that we look at our advertising costs in the large city newspapers. In these cities, we participated in theatre advertising placed by national or regional agencies. My homework showed we could save quite a bit of money by just advertising in calendar newspaper ads and discontinue the large display ads. After reviewing my information, he bought into the plan, and asked me to bring all of it to the team meeting and present it personally. Our top management and staff members liked the plan, and after consideration, we all decided to "go with it." Now, as Paul Harvey says, ”Here’s the rest of the story.” This little idea saved us thousands of advertising dollars and along with other ideas from team members, helped our company to get out of bankruptcy in record time, and I got to keep my job.

As you can see, team meetings aren’t really a waste of time if you know how to use them to help make your company better and of course in the process improve the security and benefits of your job. Using this information, you too can become a super star of your team. And last, but not least, always keep it light, smile and be happy. Smiles are contagious.
